Registration Steps
On the back side of the card, scratch to view your Serial and PIN. You will be using this to register your card.
Details to prepare:
- Personal Information and Active Contact Number
- Beneficiary Information
- Medical Background and History

Scanned Copy of Valid ID, and a Selfie with the ID. Here are the accepted Valid IDs:
- PhilSys ID
- Driver’s License
- Passport
- SSS ID or SSS UMID Card
- PRC ID
- TIN ID Card
- COMELEC Voter’s ID
- NBI Clearance
- PWD ID
- Barangay ID
Other IDs submitted is still accepted but will be validated by ETERNAL PLANS
You will be receiving message and email from us regarding the update on your contract.
This will serve as a proof of reference that you are registered as Eternal Life Planholder.
